Strains explained

Probiotics are living microorganisms that, in adequate amounts, confer a health benefit on the host (WHO/FAO 2002 definition). Most consumer probiotics fall into four families — Lactobacillus, Bifidobacterium, Saccharomyces (yeast), and the spore-forming Bacillus. They work by crowding out pathogens, producing short-chain fatty acids like butyrate, and modulating the gut-associated immune tissue (GALT). Lactobacillus and Bifidobacterium blends are the most common. The strains matter far more than the species label: Lactobacillus rhamnosus GG (LGG) has dozens of positive trials for antibiotic-associated diarrhoea, while an unnamed "L. acidophilus" strain on a generic shelf has essentially none. Total CFU is also less important than the specific strain — 5 billion CFU of LGG is more clinically useful than 50 billion CFU of unnamed strains. Saccharomyces boulardii is a yeast, not a bacterium, which means it survives antibiotics (because antibiotics kill bacteria, not fungi). The Cochrane review on S. boulardii for antibiotic-associated diarrhoea (2017, 31 RCTs, n=8,272) found a significant reduction in AAD risk (RR 0.47). It is the strongest single-strain evidence in the consumer probiotic space. Spore-based probiotics (Bacillus coagulans, B. subtilis) are shelf-stable because the bacteria form protective spores. They survive stomach acid better than vegetative Lactobacillus strains. Evidence is more modest and newer than the LGG/S. boulardii literature; megasporebiotic-class products are an emerging area but do not yet have Cochrane-grade evidence for most indications. Refrigeration is mostly a marketing signal, not a requirement. Most modern consumer probiotics are shelf-stable for 18–24 months at room temperature because the bacteria are lyophilised (freeze-dried) and the bottles contain desiccant. Products that genuinely need refrigeration will say so on the label and ship in cold-chain packaging (Visbiome is the canonical example).

  • Skip when: You're on antibiotics now (most lactobacillus strains get killed by the antibiotic).

Bifidobacterium blends

  • Mechanism: Anaerobic bacteria dominant in the large intestine. Produce acetate, support mucin layer.
  • Evidence: B. lactis HN019 and B. longum BB536 are well-studied (3/5). Most blends are mid-range.
  • Choose when: IBS-symptoms, traveller's constipation, after a course of antibiotics.
  • Skip when: You want the strongest single-strain evidence (use LGG or S. boulardii).

Saccharomyces boulardii

  • Mechanism: Yeast (not bacteria). Produces proteases that neutralise C. difficile toxins. Survives antibiotics.
  • Evidence: 5/5. Cochrane 2017 meta-analysis: 31 RCTs, n=8,272, RR 0.47 for AAD prevention.
  • Choose when: Starting or finishing a course of antibiotics, travelling to high-risk destinations, C. diff history.
  • Skip when: Severely immunocompromised, central venous catheter (rare fungemia risk).

Spore-based (Bacillus)

  • Mechanism: Spore-forming bacteria (B. coagulans, B. subtilis). Protective coat survives stomach acid; germinates in the small intestine.
  • Evidence: 3/5. Emerging, modest RCT data for B. coagulans on bloating and IBS. No Cochrane-grade reviews yet.
  • Choose when: You travel frequently and want shelf-stable without refrigeration. Better acid survival than lactobacillus.
  • Skip when: You want gold-standard evidence — pick LGG or S. boulardii instead.

How to dose

Daily maintenance. 1 capsule per day with a meal or 30 minutes before, most days of the week. Strain identity matters more than CFU count — 5 billion CFU of LGG outperforms 50 billion CFU of unnamed strains.

During antibiotics. Take Saccharomyces boulardii (Florastor or generic) starting on day 1 of the antibiotic course, 2 capsules daily spaced 2+ hours from the antibiotic dose. Continue for at least 7 days after the antibiotic ends. For Lactobacillus strains, wait until the antibiotic course is finished — most lactobacillus strains get killed by the antibiotic.

Refrigeration. Most modern probiotics are shelf-stable for 18–24 months at room temperature. If the label doesn't say "Keep refrigerated," you don't need to. Genuinely cold-chain products (Visbiome) ship in cold packs and should be refrigerated on arrival.

Course length. For specific conditions (post-antibiotic, traveller's diarrhoea), 2–4 weeks is typical. For general maintenance, indefinite daily use is safe based on available evidence. There's no documented harm from long-term use of well-characterised strains.

Empty stomach vs with food. Most modern strains are acid-resistant and don't require empty-stomach dosing. Take with food if you notice any GI discomfort.

Safety

Generally safe. Probiotics have an excellent safety record across all age groups in healthy adults. Most adverse events are mild (transient bloating or gas in the first 1–2 weeks of use). NIH ODS rates the overall safety as "very good" for healthy populations.

Immunocompromised patients. Rare cases of bacteremia or fungemia have been reported in severely immunocompromised patients (post-chemotherapy, central venous catheter, short-bowel syndrome). Saccharomyces boulardii has a specific warning for catheter patients because the yeast can colonise the catheter surface. If you fall in this group, consult your physician before starting any probiotic.

SBO / IBS severe cases. People with small-intestinal bacterial overgrowth (SBO) often feel worse on probiotics — extra bacteria in the wrong place can amplify bloating. If symptoms worsen after starting, stop.

Pregnancy and children. Lactobacillus and Bifidobacterium strains have been studied in pregnancy and infancy with reassuring safety data. Saccharomyces boulardii is generally considered safe but has less data in pregnancy. Always check with your OB-GYN or paediatrician.

Drug interactions. Probiotics can interfere with immunosuppressants. S. boulardii and antifungal medications are a specific interaction (the antifungal will kill the yeast probiotic, defeating the purpose).
