Vitamin C, explained properly.

Vitamin C is the most-supplemented vitamin on Earth — and the most-misunderstood. The dose on the label, the form (ascorbic acid vs Ester-C vs liposomal), and what you take it with make a 5-10x difference in how much actually reaches your bloodstream. This guide explains all three.

What vitamin C actually is

Vitamin C is the common name for L-ascorbic acid, a water-soluble vitamin that humans cannot synthesize. Almost all other mammals make their own vitamin C from glucose in the liver — humans (and other primates, plus guinea pigs and a few fruit bat species) lost the gene encoding L-gulonolactone oxidase about 61 million years ago, so we depend entirely on dietary intake.

This single genetic deletion is why scurvy — the historical hallmark of vitamin C deficiency — was such a devastating disease for sailors, polar explorers, and anyone without access to fresh fruit for months at a time. Today, scurvy is rare in developed countries but marginal deficiency (plasma levels < 23 µmol/L) is still common in smokers, the elderly, and people on restrictive diets.

The bottom line: vitamin C is essential, water-soluble, and tightly regulated by intestinal absorption. The body absorbs ~80-90% of a 100 mg dose but only ~10-20% of a 1500 mg dose — the rest stays in the gut and pulls water into the colon, which is why high-dose vitamin C causes loose stools and (in extreme cases) diarrhea.

How vitamin C works in the body

Vitamin C is a cofactor for 8 human enzymes. Most of its clinically-relevant effects trace back to those 8 reactions, not to the popular antioxidant framing:

  1. Collagen synthesis. Vitamin C is required for proline and lysine hydroxylation, the two reactions that make collagen fibers stable enough to hold your skin, blood vessels, tendons, and bones together. This is why severe deficiency causes bleeding gums, poor wound healing, and eventually death.
  2. Carnitine synthesis. Required to transport long-chain fatty acids into mitochondria for energy. Low vitamin C → fatigue, exercise intolerance.
  3. Neurotransmitter synthesis. Dopamine β-hydroxylase converts dopamine to norepinephrine using vitamin C. Adequate intake supports mood and stress response.
  4. Iron absorption. Reduces dietary ferric iron (Fe³⁺) to the ferrous form (Fe²⁺) that the gut can absorb. A 50 mg vitamin C dose roughly doubles non-heme iron absorption.
  5. Immune function. Concentrates in neutrophils and lymphocytes, supports phagocytosis, and shortens cold duration by ~8% in adults (1.4 days) per Hemilä 2013 Cochrane. Does NOT prevent colds in the general population.
  6. Antioxidant recycling. Regenerates oxidized vitamin E back to its active form in lipid membranes. Also scavenges free radicals directly in plasma.
  7. Carnitine + cholesterol metabolism. Affects bile acid synthesis and LDL clearance.
  8. Peptide hormone processing. Required for amidation of several hormones including vasopressin and oxytocin.

The 5 forms of vitamin C, compared

All vitamin C supplements are eventually converted to L-ascorbic acid in the body, but the starting form affects absorption, gut tolerance, and price.

1. Ascorbic acid (the standard)

Most evidence

Pure L-ascorbic acid. The form every RCT uses. Cheap, well-studied, but the most acidic form (pH ~2.4) — can irritate the gut at doses above 1000 mg.

Best for: most people at 200-500 mg/day. Buy it cheap from any AU pharmacy.

2. Sodium / calcium ascorbate (buffered)

Gentle on the gut

Ascorbic acid buffered with a mineral — sodium ascorbate or calcium ascorbate. pH closer to neutral. Easier on the stomach. The added mineral contributes a small amount to daily intake (sodium form: ~140 mg sodium per 1000 mg vitamin C).

Best for: anyone with reflux, sensitive stomach, or taking >1000 mg/day.

3. Ester-C (calcium ascorbate + metabolites)

Patented blend

A patented combination of calcium ascorbate with small amounts of vitamin C metabolites (dehydroascorbate, calcium threonate). Marketed as 2x retention vs plain ascorbic acid — the evidence is mixed, but the gentler formulation is real.

Best for: people who bloat or get loose stools from standard ascorbic acid at therapeutic doses.

4. Liposomal vitamin C

Premium delivery

Ascorbic acid encapsulated in phospholipid bubbles (liposomes). Designed to bypass the gut's transporter-limited absorption and reach higher plasma levels. A 2019 Pharmacokinetics study showed 1.8x higher plasma levels vs standard ascorbic acid at the same dose — at 4-10x the price.

Best for: people needing high-dose vitamin C who cannot tolerate the GI side effects of plain ascorbic acid.

5. Acerola / camu camu (natural source)

Whole-food

Vitamin C extracted from acerola cherries or camu camu fruit, often paired with bioflavonoids. The natural form is chemically identical to synthetic L-ascorbic acid — but the marketing "natural vitamin C is better" is unsupported by RCTs. The accompanying bioflavonoids may have small additional antioxidant activity.

Best for: people who prefer whole-food sources. Real benefits are marginal vs standard ascorbic acid at equal doses.

How much vitamin C do you actually need?

Per the US Institute of Medicine (IOM 2000) and the Australian NHMRC. The body absorbs less and less of each additional mg above ~200 mg, so dose splitting matters more than dose size for plasma levels.

The dosing strategy that actually works

Most vitamin C supplements are dosed wrong. The body tightly regulates plasma levels through intestinal absorption (SVCT1 transporter) and renal excretion. Here is what actually moves the needle:

1. Split your dose, don't megadose

Two 250 mg doses (morning + evening) give higher plasma AUC than one 500 mg dose, and dramatically higher than one 1500 mg dose. The transporter saturates around 200 mg per dose.

2. Take it with food

Vitamin C absorption improves with food in the stomach. Take your morning dose with breakfast, evening dose with dinner. Plain ascorbic acid on an empty stomach causes nausea in ~15% of people — food fixes this.

3. Don't expect to "boost immunity"

If you eat 5+ servings of fruit and vegetables daily, you are likely already saturating plasma at ~70-80 µmol/L. More vitamin C does not make your immune system stronger — it just makes expensive urine. Cochrane 2013 (Hemilä) showed no prevention benefit, only modest 8% cold duration reduction.

4. When higher doses actually help

Smokers (extra 35 mg/day). Recovery from surgery or burns (clinician-supervised 500-1000 mg/day). Iron deficiency anemia (take 250 mg with iron tablets to boost absorption 2x). Periods of high physical stress.

Smokers, sick days, and the kidney-stone myth

Smokers need more

Smoking oxidizes plasma vitamin C faster than non-smoking. NIH ODS recommends an extra 35 mg/day for smokers, equivalent to one extra orange. This is one of the few cases where higher-dose supplementation has measurable benefit.

Sick days: short-term megadosing is fine

At the onset of a cold, 1000-2000 mg/day split into 2-3 doses can shorten cold duration by ~8% per Cochrane 2013. This is a short-term strategy (3-5 days), not a daily protocol. Stop when the cold resolves.

The kidney-stone myth

Older case reports linked high-dose vitamin C (>1000 mg/day) to calcium oxalate kidney stones. The 2013 NIH large-cohort analysis found NO increased risk in men taking ≤1000 mg/day. The risk is real only for people with pre-existing kidney disease or a history of oxalate stones.

Iron-deficiency synergy

Vitamin C increases non-heme iron absorption 2-3 fold by reducing Fe³⁺ to Fe²⁺. If you have iron-deficiency anemia and are taking an iron supplement, take 250 mg vitamin C at the same meal. The iron will absorb better and your levels will rise faster.

What to pair with vitamin C (and what to avoid)

Pairs well

  • Iron (non-heme) — 2-3x absorption boost
  • Vitamin E — vitamin C regenerates oxidized vitamin E
  • Collagen peptides — vitamin C is required for collagen crosslinking
  • Bioflavonoids — citrus-derived antioxidants, modest synergy

Use caution

  • Statins + niacin — high-dose vitamin C may blunt the niacin lipid response
  • Warfarin — large (>500 mg) doses can affect INR; monitor closely
  • Chemotherapy / radiotherapy — antioxidants may interfere with treatment; consult oncologist
  • Hemochromatosis — increased iron absorption can worsen iron overload

Frequently asked questions

Does vitamin C prevent colds?

No — the 2013 Cochrane review (Hemilä, 29 trials, 11,306 participants) found that regular vitamin C supplementation does NOT reduce the incidence of colds in the general population. It does shorten cold duration by ~8% in adults and ~14% in children — but only if taken consistently, not just at onset.

Is liposomal vitamin C worth 5x the price?

For most people, no. Plasma levels plateau at ~80-90 µmol/L with oral vitamin C regardless of form. Liposomal delivery modestly raises peak plasma levels (1.5-2x in some studies) but the clinical benefit is unclear. The exception is high-dose oral vitamin C (1000+ mg/day) where liposomal avoids the GI side effects of plain ascorbic acid.

Should I take vitamin C with or without food?

With food. Ascorbic acid absorption improves with food in the stomach, and food buffers the acidity that causes nausea in some users. The exception: a fasting dose may be marginally better absorbed if you want maximum plasma spike — but the GI side effects are real.

Is natural vitamin C from acerola better than synthetic?

No, the L-ascorbic acid molecule is chemically identical whether from acerola, camu camu, or a Chinese chemical plant. The "natural is better" marketing has no RCT support. The accompanying bioflavonoids in natural-source vitamin C may add small antioxidant benefits — but the core vitamin C is the same molecule.

Can vitamin C cure cancer or COVID-19?

No to both. IV vitamin C trials (Padayatty 2006, Cameron 1979) showed no survival benefit in advanced cancer. The Linus Pauling megadose claims were never replicated. COVID-19 trials showed no prophylactic or therapeutic benefit of high-dose vitamin C in well-nourished adults. Adequate intake helps general immunity, but it does not cure serious disease.

How much is too much vitamin C?

The NIH tolerable upper limit is 2000 mg/day for adults. Below that level, side effects are rare. Above 3000 mg/day, loose stools and diarrhea are common — vitamin C pulls water into the colon via osmosis. Above 5000 mg/day, nausea and abdominal cramps are typical. Stop when symptoms resolve; there is no long-term harm from a single high dose.
